They are soon going to require you to fill out the immigration form to enter the Dominican Republic online as of the end of the year

I gave it a try and it seems quite buggy
But what I figured out is that when you get to the page to enter your information you need to change the language to english or not all of the drop lists will work properly and you won't be able to complete it all.
With that change the input will go much smoother and at the end it will generate a pdf with a QR code that you can save and print out to take later
If they accept it as is it should make things a little smoother.
It does ask some health questions.

https://eticket.migracion.gob.do/Colombia just adapted this new online method a few months ago. Except Colombia is not filled with a population of 6th graders so no bugs.

It combines all three forms into one now so much less to complete
This single form replaces the three forms currently required: International Boarding and Disembarkation Card,
Customs Declaration Form,
and Traveler's Health Affidavit. Now all this information is in one place.
And it can be completed anytime after you purchase your flight
Seems like it makes the traveler's health affidavit pretty much useless but I guess fill it out when you are feeling good lol

No that is not the problem
Make sure you have it set for Arrival in the first step rather than Departure.
You only need the health information for coming into the DR
It is kinda buggy so if you are not seeing the information in the drop down lists displaying than the site is not loading properly and you won't be able to get it to work
Try switching between English and Spanish and back to English. You might have to refresh the screen but that method seems to get it working again when the drop down lists are not showing up
Those drop down lists have to show up or you won't be able to get through it all
it would make sense that you fill out the health declaration form close to your departure but that does not seem to matter.

Has anyone used this online form yet?
I filled mine out and it won't go to the #4 section about public health. Is this because I am not close enough to my departure?It was flawless when completed on my laptop. But definitely buggy when using a phone. I had to refresh and start over 12 times. I'd get to a certain screen and the drop down wouldn't function. I don't recommend using a phone or crappy wifi.

I was finally able to to do the arrival form and printed out the bar code. I see now you don't need the customs and health info for departure but I didn't see a spot to print out a barcode for the departure. Do I need another barcode or is all the info in the one I printed?

You will need a separate one for the departure and you are right there won't be any customs information or the health declaration when you leave.
Think about when you arrive there is the immigration form, the customs declaration and now the health form But when you leave there is only the immigration form it's the same for the online version.
I guess they don't care about how you feel when your leaving as that will be another countries problem.

Hell no and I would do everything possible to hide it if I wasn't feeling well.
Obviously I would do whatever possible to protect others but no way would I want to open that can of worms.
The last thing I would want is to be not feeling well and stuck in that country.
At POP you have to go through a temperature check prior to entering the airport and they ask you some health questions at the flight desk.

Right but if you do your likely screwed.
Although I did read that there was someone that failed a temperature scan and ended up testing positive and they still put them on the plane.
I guess we realize that we are taking some risk when traveling right now but depending on where you go and how you handle yourself you might actually be safer in another country than some places in the USA right now
That's the tough thing with this virus. You just don't know who has it and what your chances are of getting it or how it will affect you.

enter that code and it will take you directly to the page with the form and the code should be there so you can print it out
Instead of selecting APPLY FOR select TO ACCESS and you will be able to enter your access code and security answer you setup

I am using Safari on my MacBook Pro.
